Unlisted basically means anyone can see it, but it doesn't show up in search results or trends or followed hashtags.

Some customised versions of Mastodon have a "Local" visibilty which only people on your instance can see, but this option is not available on standard Mastodon.

Your instance would have to be using Hometown Mastodon or some Glitch Mastodon instances to have the option of Local visibility.
